The Problem We Solve
Hospitals are discharging patients home sooner and sicker while Home Health Agencies struggle with limited physician access, documentation gaps, and growing readmission risk. Families are left juggling medications, appointments, and complex instructions without consistent medical support in the home.
Gaps in Physician Access
Primary care physicians may be out-of-state, unavailable, or unable to support frequent in-home needs, leaving HHAs without a reliable partner to sign orders or guide complex care plans.
Documentation & Payment Risk
485s, face-to-face documentation, and orders are often delayed or incomplete, putting agency cash flow, compliance, and staffing stability at risk.
Avoidable ED Visits & Readmissions
Without timely intervention in the home, manageable changes in condition escalate into emergency visits and hospital readmissions that strain patients, families, agencies, and hospital systems.
Our Transitionalist Solution
Winbigler Medical acts as a dedicated transitionalist medical group, partnering with Home Health Agencies to stabilize and support patients in the home using integrated telemedicine, strong hospital relationships, and real-world home health experience.
Transitionalist Medical Group
We function as a physician-led transitionalist team focused on the critical window after hospital discharge, partnering directly with HHAs to keep patients stable, supported, and out of the hospital.
Telemedicine Integrated with Home Health
Physicians collaborate with nurses in real time via telemedicine, supporting medication changes, symptom escalation, and care plan adjustments while patients remain safely at home.
Designed Around Real-World Home Health
Built by a hospitalist who rounds in the hospital and a team that understands home health operations, our model aligns with how HHAs actually work — not how a generic telehealth company thinks they should work.
How Winbigler Works with Your Home Health Agency
We plug into your existing workflows as your transitionalist medical partner, supporting nurses, stabilizing patients, and protecting your revenue cycle.
1
Partner Onboarding & Workflow Mapping
We learn your agency’s referral patterns, service area, clinical mix, and documentation processes, then align our physicians and telemedicine tools to fit your team’s daily reality.
2
Integrated Clinical Support
Our physicians collaborate with your nurses, provide telemedicine visits, sign orders, and address changes in condition — all with a focus on preventing avoidable readmissions.
3
Stabilized Patients, Stronger Agencies
With fewer readmissions, better documentation, and stronger relationships with hospitals and families, your agency can grow confidently and sustainably.
